1. What is a Physical Therapist?
A Physical Therapist is a licensed health care professional. They help restore function, improve mobility, relieve pain, and prevent or limit permanent physical disablilities of patients suffering from injuries or disease.

4. What training does a Physical Therapist have?
A Physical Therapist has completed an acredidted 4-6 year college progam. They have met the educational requirements in physical therapy, including studies in biology, basic medical sciences, and clinical experience. Physical Therapists must pass an examination and receive a license to practice.
5. How can I be treated by a Physical Therapist?
Physical Therapists will often need a diagnosis from a licensed health care professional who is authorized by his or her license to diagnose (i.e. physiciance, dentists, podiatrists, chiropractors, etc.) A Physical Therapist may evaluate a patient without a diagnosis; one is required prior to Physical Therapist providing any treatment. Once you have a diagnosis, the Physical Therapist will implement a personalized treatment plan for you.
6. What should I bring to my first Physical Therapy appointment?
- Your Insurance Card and Drivers License
- Perscription from your physician for Physical Therapy
- Co-payment based on your insurance plan
7. What time should I arrive for my first appointment?
It is recommended that you arrive 15-20 minutes prior to your scheduled appointment time to complete all the necessary paperwork.
8. What should I expect to take place during my first visit?
An initial evaluation will be performed by a Physical Therapist. This appointment will involve some detailed questions to answer about how you are feeling, and setting up your goals and treatment plan. After your evaluation is completed, some portion of the treatment plan will be started on the first visit.
There is always ample time alloted to answer any questions you may have regarding your condition. At the conclusion of your visit, you will be asked to schedule follow-up appointments based on your needs and availability with the front office staff.

10. How do I get to Free Motion Rehabilitation Center?
From Freehold Area and North
Take Route 9 South past Route 195. Look for the Kmart Shopping Center on your Right. At the Traffic Signal at the end of the Kmart Parking Lot make a RIGHT onto New Friendship Road. Take the First Left into Friendship Mall. Free Motion Rehab is located in the middle of the shopping center straight ahead.
From Lakewood Area and South:
Take Route 9 North past the Kohl's Shopping Center. Stay in the Right Lane. At the Traffic Light for New Friendship Road, take the Jughandle (right after the light). The Southard Firehouse will be on the corner on your right. Cross over Route 9 at the Jug and go directly across onto New Friendship Road. Take your first left into Friendship Mall. Free Motion Rehab is located in the middle of the shopping center straight ahead.
